Our skilled rehabilitation program offers a wide range of short-term
rehab and medical services for orthopedic, cardiac, neurological,
pulmonary and medically complex conditions. The goal is to provide
the highest quality patient care while facilitating a quick and safe
transition from the hospital to our health center and then to your home.
When the patient is ready to leave, we provide support and make referrals
to service providers.
When a patient has chosen to live with us, we can assist them with
transitioning into our long term care unit; from room selection to
assistance/guidance with Medicare/Medicaid. Through our Family Approach
to Caring and experience we are able to provide a lively and caring
community with a staff that diligently addresses residents' needs
promptly. Our residents are continually assessed for changes in
conditions and their plan of care is constantly updated to reflect those
changes. Our plan of care always reflects the resident's individual and
personal choices. Quality of life and maintaining the highest level of
